How to register with Melbourne and Chellaston Medical Practice
You can attend the surgery and complete a registration pack and GMS1 form or you can print the following forms and bring them to the surgery along with 1 x photo ID and 1 x proof of address (if you do not have photo ID, please bring in 2 x proof of address) for each family member. If you are registering a Child please bring in their red book, if they have one.

Registering with the surgery
To register with the surgery:
- fill out an online registration form
- visit reception from 10am to 6pm and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
