Requesting Sick/Fit note

Sick/Fit note requests less than 7 days

If you have been ill for 7 days or less you do not need to see a Doctor for a sick note. You can complete a self -certification form. Many employers have their own self-certification forms.

Please remember you will need to hand a copy of your self-certification form to your employer. You do not need to see a Doctor. 

Sick /fit note request more than 7 days

If you are off work for more than 7 days, your employer will normally ask you to provide a medical certificate from your Doctor.  If you need to request a sick note, you will need to call the practice to book an appointment to speak to a GP.
