While this information may be utilized for administrative and quality assurance purposes, it is primarily accessible to and used by individuals directly involved in your healthcare. You have the right to access and review the information we have on file for you. If you wish to view your records, please get in touch with our Practice Manager.
We prioritise your privacy:
Rest assured that any conversations you have with any member of our practice, whether it’s your family doctor, nurse, or receptionist, will remain confidential. Even if you are under the age of 16, we will not disclose any information to anyone, including parents, family members, caregivers, or educators, without your explicit consent. The only exception to this would be if there is a compelling need to share confidential information to prevent serious harm to yourself or someone else, and in such cases, we would make every effort to discuss this with you beforehand.
If you are receiving treatment elsewhere, such as at a hospital or a Brook center, we recommend allowing the doctor or nurse to inform our practice about the treatments you are undergoing to ensure seamless coordination of your care.
